SBL Dental forms will be sent out August 19 & 20, 2024. Parents who would like their child to be seen by the Dental Bus need to fill out the form completely and return to school no later than August 30, 2024. Any forms after this time will be put at the bottom of their list and will only be seen as their schedule allows.
The DENTAL BUS fulfills the dental requirement for Kindergarten, 2nd, 6th and 9th grade students! Complete every question on the form. If something is left blank there is a chance the student may miss this opportunity.
